- Original work permit / Copy of labour card receipt/Company Labor card copy/ Copy of e-signatory card of Ministry of Labour/copy of labour contract (if applicable)
- Copy of the fitness certificate/medical test report (if applicable)
- Two photographs of the employee/family member with white background (whichever applicable)
- Phone numbers, P.O Box number, office number, Building name, street name etc. (whichever applicable)
- Dewa Bill,Telephone bill,Tenancy contract,Location map of establishment etc
